Orientation to College

Orientation Surveys

The following surveys are mandatory for all students enrolling in either the Distance or On-Campus Orientation courses.  Upon submission of each survey, you will be asked to print a success page as proof of completion.  Your instructor will ask for these pages when you turn in your orientation handbook.

Attention Fall 2009 students: We have reached the limit on the number of submissions in our online form system for the month of September. If you have not already completed your surveys, please complete the surveys and print them without submitting to turn in to your instructor.
