Orientation Surveys

The following surveys are mandatory for all students enrolling in either the Distance or On-Campus Orientation courses.  Upon submission of each survey, you will be asked to print a success page as proof of completion.  Your instructor will ask for these pages when you turn in your orientation handbook.

• Student Services Survey (Required for All Orientation Students)

• Distance Education Quiz (This is an optional quiz. However, if you are interested in distance learning, this is a helpful tool to help you decide if this way of learning will work for you.)
